I understand that tattooing is artistic in nature, and no results or expectations can be guaranteed.

I understand that a tattoo is a permanent change to my appearance and can only be removed by laser or surgical means. I understand that the removal process may lead to scarring or disfigurement, and can be costly. I understand that after receiving the tattoo, my skin may never be restored to its exact appearance prior to the tattoo.

I have answered every question thoroughly and honestly, and understand that withholding information or lying may have negative repercussions on the treatment and/or results.

Disclaimer: Please be advised that our company cannot be held responsible for any damage, including but not limited to stains or any other form of damage that may occur to your clothes during any treatment. While we strive to ensure a safe and professional environment, accidents can happen, and we cannot accept liability for any such incidents. We kindly request that you take necessary precautions to protect your garments during the treatment or wearing stain proof dark clothing to your appointment. By paying your deposit and participating in the treatment, you acknowledge and agree to release our company from any liability or claim related to damaged clothing.
